EQUIP FOR FUNDING GRANTS

The Equip For Funding Grant Program is open to adults with special needs. It is a grant program for equipment, therapy, or activities that are not covered by insurance to help keep them independent and active.

ELIGIBILITY CRITERIA

  • Diagnosis - Physical or Developmental

  • 21 years of age or older (For those individuals under the age of 21, please visit the Robbie Foundation) https://robbiefoundation.com/apply

ABOUT THE APPLICATION PROCESS

The Equip For Living Foundation is a volunteer-run organization and applications are reviewed annually by a committee. Incomplete applications will not be considered. 

The Equip For Living Foundation reserves the right to fund requests based on priority and necessity.

You will hear from Equip For Living by the end of the month following the deadline only IF you have been approved. Due to the high volume of applications we will not contact you if your application has not been approved. 

Deadlines for application submissions are as follows: 

  • September 30, 2023

How to Apply

An application detailing information about the adult in need of assistance and the reason for the need is required. Funding will be given for items and services that help maximize growth and development, as well as create environment for the individual to be healthy, independent and engaged in an active lifestyle.

Please read carefully before you start the application process.

  • Application deadline: September 30, 2023.

  • Any individual may submit an application on behalf of a recipient. Consent must be given by individual or parent(s)/guardian(s).

  • When applying for an item typically funded by insurance or MaineCare, you will be required to submit the denial letter. Otherwise your application will not be considered.

  • Although it is no longer a requirement, grants are prioritized for individuals who live in the state of Maine, have cerebral palsy, or other congenital disabilities. 

  • Before you begin, gather the following: personal information, letter of recommendation, a quote for the item being requested and insurance or MaineCare denial letter (if applicable). Acceptable formats for documents to be uploaded include: PDF, JPG and Word.

  • To ensure we received your application: You will reach a confirmation page stating that your application was successfully submitted. The confirmation page has a checkmark and states: Thanks for sharing this information with the Equip For Living Foundation.

  • Before submitting, please keep a copy or print-out of the application for your records.
